If you would like your story to be part of this ongoing project, please read the following:

Below are some questions to consider when describing your experience: please do not feel you have to address all, if any of them. Length is up to you (2-10 pages is suggested.) If you want to use a pseudonym or be anonymous, please indicate this on your submission.

Personal information will only be used to be in touch with you about your submission. Thank you for making a difference in the mental health community by contributing to this collection! When complete, it will be available through the Cole Resource Center at McLean Hospital, Belmont MA. Some selections will also be available on-line at this website.

Describe your life:

• What was life before you were diagnosed?
• How has life been since you were diagnosed? What treatments, structures and supports have helped to maintain your health and well-being?
• Medications (you don't have to say which ones)?
• Other treatments?
• Therapy (talk, group, cognitive behavioral, etc.)?
• Family and/or Friends?
• Support Groups?
• Lifestyle Choices (diet, exercise, etc.)?
• Practices (some daily routine, religious, meditation, etc.)?
• Work, Education or Programs?
• Volunteer Opportunities and/or participating in activities where you feel you make a difference? •Other?

Graphic Files

If you are submitting graphic files they must meet the following requirements:

• File size must be less then 200 kilobytes.
• Be in the corm of GIF or JPEG standards.
• Be of an expectable resolution for use in an online format (please test on your own computer first and insure that you are at lease using the 256 standard colors).
